AbstractMad honey poisoning occurs after ingestion of honey including grayanotoxin. Grayanotoxin shows its effects by binding to Na channels expressed at the cell membrane. In this case, we aimed to present the diagnosis and treatment of mad honey poisoning which is a clinical picture hard to differentiate from other diseases due to its nonspecific symptoms and findings.A 64 year old male presented to emergency department with complaints of nausea, vomiting, dizziness and syncope. On his examination, tension arterial was 70/45mmHg, pulse was 53beats/minute and respiration rate was 14/minute. His ECG revealed sinus bradycardia. The patient had a history of eating 2 tablespoons of mad honey approximately one hour ago. He was treated with 1 mg of atropine intravenous and 1000 ml of saline infusion. He was discharged after 8 hours of observation. Emergency physicians should consider mad honey poisoning if a patient presented to emergency department with complaints of nausea, vomiting and bradycardia. |

ÖzetDeli bal zehirlenmesi, grayanotoksin (GT) içeren balın yenilmesiyle oluşmaktadır. Balın içinde yer alan GT?ler etkilerini hücre membranlarındaki sodyum kanallarına bağlanarak göstermektedir. Acil servise başvuran hastada spesifik olmayan şikayet ve bulgular sebebiyle bir çok hastalıkla karışabilecek deli bal zehirlenmesinin tanı ve tedavisini sunmayı amaçladık.Altmış dört yaşındaki erkek hasta acil servise uzun bulantı, kusma, baş dönmesi ve senkop şikâyetleri ile geldi. Fizik muayenede; arteriyel basınç 70/45 mmHg ve kalp tepe atımı 53 atım/dak, solunum sayısı 14/dk idi. Elektrokardiyografide sinüs bradikardisi vardı. Yaklaşık 1 saat önce 2 çorba kaşığı deli bal yediği öğrenildi. Hastaya 1 mg atropin ve 100 ml serum fizyolojik verildi. 8 saatlik izlem sonrasında hasta taburcu edildi. |
