AbstractSpontaneous diaphragm rupture is a rare condition. It is usually observed at left hemidiaphragm. Rupture of right-sided diaphragm, which is an extremely rare condition, is in general observed following severe trauma.A 21 year-old female undergrad presented our emergency room with acute dyspnea. Neither trauma nor infection were noted in her history. Pleural fluid was noted at right hemithorax on plain chest x-ray. The fluid was drained via tube thoracostomy. Urgent thoracotomy was applied as the fresh hemorrhagic drainage exceeded 500 ml. Spontaneous right hemidiaphragm laceration of grade III was detected. As the hematoma was drained, diaphragm was repaired primarily. In result of this case report, rupture of right hemidiaphragm should be included within differential diagnosis of patients presenting with acute spontaneous dyspnea and righ-sided hemothorax. Definite diagnosis and management of spontaneous rupture of right hemidiaphragm without organ herniation can only be achieved via surgical approaches. |

ÖzetSpontan diyafragma rüptürlerine nadir rastlanılmaktadır. Diyafragma rüptürleri sıklıkla sol hemidiyafragmada gelişir. Sağ hemidiyafragma rüptürü oldukça nadir olup yüksek şiddetli travmalarda gözlenmektedir.21 yaşında bayan üniversite öğrencisi akut dispne ile acil servise başvurdu. Anamnezinde travma , enfeksiyon öyküsü yoktu. Çekilen toraks grafisinde sağ hemitoraksta plevral sıvı izlendi. Plevral sıvı tüp torakostomi ile drene edildi. Taze hemorajik drenaj sıvısı 500 cc? den fazla gelmesi üzerine acil torakotomi uygulandı. Grade III sağ spontan diyafragma laserasyonu saptandı . Hematom drene edildi ve diyafragma primer onarıldı. Bu vaka sunumu ile spontan gelişen akut dispne ve sağ spontan hemotoraks ile başvuran hastalarda ; sağ diyafragma rüptürü de tanıda düşünülmelidir. Organ herniasyonu olmaksızın spontan sağ diyafragma rüptürlerinin kesin tanı ve tedavisi cerrahi ile mümkündür. |
