A Case of Intracranial Mass Lesion Presented with Isolated Oculomotor Nerve Paralysis

Abstract

Oculomotor nerve palsy occurs uncommonly in children. An eight-year-old girl was admitted to our outpatient clinic with left eye lid drop. Her neurological examination showed ptosis of the left eye, mydriasis, down and out deviation of the left eye. Her cranial magnetic resonance imaging showed contrast enhancement in the oculomotor nerve and pons. Methylprednisolon therapy was started due to suspected infectious etiology in a center. Her symptoms disappeared under the treatment first, however they reoccured. The patient was hospitalized for further investigation. As the steroid therapy was tapered she had right hemiparesis. Her repeated cranial magnetic resonance imaging showed hyperintensity in pons and contrast enhancement prominent on the left side. She was diagnosed with pons glioma.

Özet

Çocukluk çağında okülomotor sinir felci nadir görülür. Sekiz yaşında bir kız çocuğu sol göz kapağında düşüklük ile başvurdu. Nörolojik muayenesinde sol gözde aşağı dışa bakış, pitoz ve midriyazis mevcuttu. Beyin manyetik rezonans görüntülemesinde sol okülomotor sinir ve ponsta kontrast tutulumu enfeksiyon lehine değerlendirilerek hastaya bir merkezde metilprednisolon tedavisi başlanmıştı. Tedavi altında yakınmaları gerileyen ancak bir süre sonra yineleyen hasta ileri tetkik için hastaneye yatırıldı. Steroid tedavisi azaltılırken sağ hemiparezisi gelişti. Beyin görüntülemeleri tekrarlanan hastada solda daha belirgin olmak üzere ponsta silik sınırlı sinyal artışı ve kontrast tutulumu saptandı. Hasta pons gliomu tanısı aldı.
