AbstractAfter a blunt trauma, by a sudden increase in intraoccular pressure orbital wall fracture results with fractures in the thin walls. In children, the most common type of blow-out fracture is trapdoor. Blow-out fracture occurs most often in the inferior (maxillary) and medial (ethmoid) wall. In the inferior wall fracture limitation glance up, enophtalmus, ptosis, pain at the cheek on the same side and diplopia occur because of inferior rectus muscle tightness. A 12-year-old girl was brought to emergency department with the left eye and facial injuries after facial trauma, and then for diplopia and limitation of upgaze maxillofacial computed tomography (CT) was performed. The CT findings showed trapdoor orbital floor fracture. After medical treatment, on the third day after the trauma, the patient was operated. This case is presented to emphasize the importance of surgery performed during the early stages of trauma and not to overlook this type of fracture. |

ÖzetOrbital duvar kırıkları, künt travma ile oluşan intraoküler basıncın ani yükselmesi nedeni ile meydana gelir. Çocuklarda en yaygın olarak trapdoor tipi blow-out kırığı görülür. Blow out kırıkları en sık inferior (maxiller) ve medial (etmoid) duvarlarda meydana gelir. İnferior rektus kası sıkıştığı için inferior duvar kırıklarında yukarı bakış kısıtlılığı, enoftalmus, pitozis, aynı tarafta yanakta ağrı ve diplopi görülür. 12 yaşında kız hasta sol göz ve yüz travması sonrası acile getiriliyor, hastada diplopi ve yukarı bakış kısıtlılığı olması üzerine çekilen maksillofasiyal bilgisayarlı tomografi (BT)?de orbita tabanında trapdoor şeklinde fraktür saptanıyor. Hasta medikal tedavi sonrası, erken dönemde travma sonrası üçüncü gün cerrahiye yönlendirilerek opere edildi. Bu tip kırıkların gözden kaçırılmaması ve erken dönemde yapılan cerrahi operasyonun önemini vurgulamak amacı ile bu vaka sunulmuştur. |
