False Positive F-18 FDG Finding in IgG4-related Hashimoto’s Thyroiditis

Abstract

A 47-year-old male admitted to our clinic with obstructive symptoms on his neck. He had no history of hyperthyroidism and no co-morbid disease. On physical examination, it was found that the thyroid gland was enlarged, stiff, irregular in contours, and partially fixed to the surrounding tissues by palpation. Lymphadenomegalies of 1 cm diameter were detected in the right cervical third (3rd) zone. Ultrasonography (USG) detected an enlarged and widely heterogeneous thyroid gland and a lymph node in malignant appearance in the right cervical 3rd region. Laboratory examinations revealed hypothyroidism with high anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-Tg) levels in addition to elevated beta-2 microglobulin and immunoglobulin G4 (IgG4) levels. A diagnosis of lymphoplasmacytic cell infiltration was made as a result of core needle biopsy accompanied by USG for tissue sampling. Upon detection of increased Fluodeoxyglucose-18 F (18-FDG) uptake in the thyroid and a cervical lymph node on Positron-Emission Tomography and Computed Tomography (PET / CT), the patient was performed isthmusectomy and lymphadenectomy to make a definitive diagnosis and relieve pressure on the trachea. The final pathological diagnosis was reported as IgG4-related Hashimoto’s thyroiditis. Herein, it is aimed to present a rare case of IgG4-related Hashimoto’s thyroiditis and its differential diagnosis.

Özet

47 yaş erkek hasta boyunda bası semptomları ile kliniğimize başvurdu. Hikayesinde hipertiroidi bulguları olmayan hastanın ek yandaş hastalığı olmadığı öğrenildi. Fizik muayenede tiroidin büyümüş, sert, konturları düzensiz, palpasyonla çevre dokulara kısmı fikse olduğu saptandı. Sağ servikal 3. bölgede 1 cm'lik lenfadenomegaliler tespit edildi. Ultrasonografi (USG) incelemesinde tiroid bezi büyümüş ve yaygın heterojen yapıda saptandı. Sağ servikal 3. bölgede ise malign görünümde lenf nodu olduğu görüldü. Laboratuvar tetkiklerinde hipotiroidi, anti-tiroid peroksidaz (anti-TPO), anti-tiroglobulin (anti-Tg) yüksekliği ile beta-2 mikroglobulin ve immunglobulin G4 (IgG4) yüksekliği tespit edildi. Doku örneklemesi amacı ile yapılan USG eşliğinde kalın iğne biyopsisi sonucunda lenfoplazmositer hücre infiltrasyonu tanısı alındı. Bunun üzerine çekilen Pozitron Emisyon Tomografisi/Bilgisayarlı Tomografi(PET/CT)’de tiroidde ve servikal lenf nodunda artmış F-18 Florodeoksiglikoz (18F-FDG) tutulumu saptanması üzerine tanısal ve hava yolunu açmak amaçlı isthmusektomi ve lenfadenektomi yapıldı. Nihai patolojik tanı IgG4 ilişkili Hashimoto tiroiditi olarak raporlandı. Bu yazıda nadir görülen IgG4 ilişkili Hashimoto tiroiditi ve ayırıcı tanısının sunulması amaçlanmıştır.
