AbstractMarginal ulcers occur along the gastroenterostomy line. We report a case of marginal ulcer perforation in a 45-year old male presented with peritonitis and a background of surgery for peptic ulcer disease 5 years ago. The patient had a smoking history and use of non-steroidal anti-inflammatory drug. There was no other disease known in the patient's past. The patient was taken to emergency operation and there was 1x1 cm perforation on the afferent loop of of the anterior gastrojejunostomy line and extensive staining of the peritoneal cavity with bilious fluid. Repairs were done with the falciform ligament which is a rare practice. The patient did not have any problems in the postoperative period. The repair of the perforation with the falciform ligament is a cheap and easily applicable method that is an alternative to omentopexy. |

ÖzetMarjinal ülserler, gastroenterostomi hattında ortaya çıkan ülserlerdir. Biz, bu yazıda, peptik ülser hastalığı için 5 yıl önce cerrahi öyküsü olan ve peritonit ile başvuran 45 yaşındaki erkek hastada ortaya çıkan marjinal ülser perforasyonunu sunduk. Hastanın non-steroidal anti-inflamatuar ilaç kullanımı ve sigara içiciliği öyküsü vardı. Hastanın özgeçmişinde bilinen başka bir hastalığı yoktu. Hasta acil operasyona alındı ve gastrojejunostomi hattının anteriorunda afferent ansta 1x1 cm perforasyon ve peritoneal boşluğun safralı sıvı ile yaygın kirlenmesi mevcuttu. Hastaya nadir bir uygulama olan falciform ligaman ile onarım yapıldı. Postoperatif süreçte hastanın herhangi bir problemi olmadı. Falciform ligaman ile perforasyon onarımı, omentopeksiye alternatif olarak ucuz ve kolay uygulanabilir bir yöntemdir. |
